We will need the following information in order to set up your business account. Please provide a copy of the following:

1. Retail Business License
2. State Tax License
3. Federal Tax ID Number
4. Store Name and Telephone Number
5. Completed Terms, Condition and Agreement From, Credit Application

Orders placed before 11:00am sill be in most cases shipped out same day depending on the state.
